Mandatory TB Screening Form

All incoming students (undergraduate, graduate, and transfer) must complete the form linked below and return to SHS as instructed. The form can be returned using 1 of the following methods:

• Fax to SHS at 801.832.2247
• Scan and email to studenthealthservices@westminstercollege.edu
• Drop off at SHS during clinic hours
• Mail to Westminster College – Student Health Services, 1840 South 1300 East, Salt Lake City, UT 84105

This form should be completed prior to the first day of school. If not received by the second month of the current semester, a hold may be placed on your school account.
